POSITION SUMMARY:
 
The Cardiac Nurse Practitioner works in collaboration with a physician, and other hospice care team members, to deliver hospice care and related health care services to patients and their families. We offer competitive salary, excellent benefits, and lots of room for growth (55 locations).

QUALIFICATIONS:
 

  • Graduate of an accredited school of professional nursing. 
  • Masters Degree in Nursing and completion of an Accredited Nurse Practitioner Program.
  • Current RN licensure and certification to practice as an advanced practice nurse by state board in the state of practice.
  • 2 - 5 yrs of current experience, specifically in Cardiac Care.
  • Excellent nursing judgment and assessment skills.  Excellent  documentation, oral and written communications and interpersonal skills.
  • Knowledgeable in pain and symptom management.
  • Demonstrated clinical competency in applying principles, methods and equipment used in nursing and appropriate specialty.  Identified interest to work in Cardiac Care.
  • Current CPR certification preferred.
  • Demonstrates sensitivity to the needs of the patients and caregivers (families, friends and significant others) with the ability to function as an effective member of an integrated healthcare team.
  • Ability to work flexible hours.  Must be a licensed driver with reliable, insured transportation (in accordance with organization policy).
  • RESPONSIBILITIES AND DUTIES:
     
  • Promotes advanced nursing practice in the hospice and palliative care setting by: directing and evaluating patient care; providing consultations to the nursing staff; collaborating with the medical staff of hospice and in the community; maintaining professional knowledge and skills; and acting as a role model for other nurses.
  • Initiate palliative care consultations or appropriate hospice treatment plans for patients in all settings.
  • Assist with determination of hospice eligibility and collaborates with the nursing staff in implementation and revisions to plans of care as necessary. 
  • Collaborates with the nursing staff, medical staff and patient/family in providing pain management and specialized nursing skills related to end of life care.
  • Prepares history and physical, clinical and progress notes that demonstrate progress toward established goal(s).
  • Completes all documentation as required by organization in compliance with legal and governmental requirements.
  • Consults with and educates patients/family regarding: disease process, self-care techniques, end-of-life care, the processes for dealing with issues of ethical concern.
  • Provides specialized hospice training to family members and informal caregivers and assists with/recommends appropriate referrals for other professional interventions when necessary. 
  • Participates in Quality Assessment and Performance Improvement activities.
  • Maintains ongoing communication with attending physician/designee/medical director and the Clinical Coordinator and communicates information using current process and technology available to organization.
